CHANGE OF NAME (involves issue of New Passport Booklet)

Application to be filled by following instruction on

3. Current Passport in original with photocopy of first 2, last 2, address page if on separate page, any other
endorsement pages and visa page with additional booklets if any.

4. Documentary evidence of change of nameas follows: -

(I)Clippings of two English newspapers viz; one local and one Indian or the Gazette Notification of the concerned State Government, as the case may be;

(II)Two Indian public/school documents issued in the desired /applied changed name to ascertain that the applicant has actually changed his/her name.  

(III)  In case of applicants residing abroad continuously for over 3 years and having Permanent Residence/work permit/visa in the country, two public documents in the changed name issued by concerned foreign government or foreign school where the minor child is studying. This will not be applicable in respect of applicants who are on tourist/visit visas or temporary visas.

5. Documentary evidence of change of name due to marriage, remarriage or divorce as follows: -
(I). Except the details provided by the applicant in the application form, no proof of marriage/dissolution of marriage (court’s order for judicial separation/decree of divorce) etc. is required”

6. If the validity of the present passport expired more than 6 months, ‘Sworn Affidavit’ for late submission,  signed by applicant in the presence of Consular Officer.

7. Personal appearance of applicant is compulsory to establish physical identity.

CHANGE OF ADDRESS (involves issue of New Passport Booklet)

1. Application to be filled by following instruction on

3. Current Passport in original with photocopy (of first 2, last 2, address page if on separate page, any other endorsement pages and visa page with additional booklets if any.

4. Documentary evidence of change of address like Nativity Certificate in original issued by District Collector /Tehsildar /Commissioner/ Panchayat or copies of Aadhar card, ration card, Election Commission ID card, Title deed, water, electricity or telephone bill in applicant’s name. In case new address involves change of State, the documentary evidence being produced should be attested by the Home Department of the State Government concerned. For minor’s parent’s passport copy is accepted as a proof.

5. If the validity of the present passport expired more than 6 months ago, Sworn Affidavit for late submission, signed by applicant in the presence of Consular Officer.

6. Personal appearance of applicant is compulsory to establish physical identity.

POLICE CLEARANCE CERTIFICATE (PCC)

3. Current Passport in original with photocopy (of first 2, last 2, address page if in separate page, any other endorsement pages and residence/employment visa page with additional booklets if any.)

4. Copy of Latest Police Clearance Certificate in English obtained from UAE Authorities (Original to be produced for verification).

5. Personal appearance of applicant is compulsory to establish physical identity.

CANCELLATION OF ECR ENDORSEMENT (Involves issue of New Passport)

The individuals who are eligible for cancellation of ECR Status are:-

a. All holders of Diplomatic/official passports.

b. All Gazetted Government Servants.

c. All Income-Tax payers (including Agricultural Income-Tax Payees) in their individual capacity. Proof of assessment of Income-Tax and actual payment of Income Tax for last one year or copy of PAN card to be insisted upon, and not merely payment of advance tax. However, in most cases as an assessment order is not issued separately by the Income Tax department, Income Tax Returns which are stamped by Income Tax authority can be accepted.

d. All professional Degree holders, such as Doctors holding MBBS Degree or equivalent degree in AYURVED, or Homeopathy, accredited journalists, Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, scientists, Advocates, etc.

e. Spouses and dependent children of category of persons listed from (b) to (d).

f. All persons having educational qualification of matriculation and above.

g. Seamen who are in possession of CDC or Sea Cadets, Deck Cadets:

h. who have passed final examination of three year B.Sc. Nautical Sciences Course at T.S. Chanakya, Mumbai and

i. who have undergone three months pre-sea training at any of the Government approved Training Institutes such as T.S. CHANAKYA, T.S. REHMAN, T.S. JAWAHAR, MTI(SCI) and NIPM, CHENNAI after production of Identity cards issued by the Shipping Master, MUMBAI/KOLKATA/CHENNAI.

j. Persons holding permanent Immigration Visa, such as the visas of UK, USA and Australia.

k. Persons possessing two years diploma from any Institute recognized by the National Council of Vocational Training(NCVT) or State Council of Vocational Training (SCVT) or persons holding three years’ diploma/equivalent degree from institutions like polytechnics recognized by central/state Governments.

l. Nurses possessing qualifications recognized under the Indian Nursing Council Act, 1947.

m. All persons above the age of 50 years.

n. All persons who have been staying abroad for more than three years (the period of leave / stay in India to be deducted).

o. All children up to the age of 18 years of age. (At the time of re-issue at the age of 18 years, ECR stamping shall be done, if applicable).

NRI CERTIFICATE 

1. Application in EAP-II form

3. Valid Passport in Original (which should cover the whole previous Financial Year, i.e. from 1st April, if not previous passport also) with photocopy of first, last, address page if on separate page, any other endorsement pages and visa page with additional booklets if any.)

4. An affidavit by the applicant to the effect that he has not stayed in India for more than 182 days during the PREVIOUS Financial Year (1ST April,….. to 31st March, ……).(Format enclosed)

5.  If the applicant wants to have the name of his/her son/daughter mentioned in the NRI certificate, then the applicant should give a copy of Passport (1st and last pages) of the child. [The names of other relative of the applicant will not be mentioned in the certificate.]

SURRENDER/ RENUNCIATION OF INDIAN PASSPORT

3. Valid Passport in original, along with previous Indian passports if any, and photocopies of pages first two, last two and pages containing valid visa and other observations.

4. Valid foreign passport, along with previous foreign passports if any, and photocopies of pages first two, last two and pages containing valid visa and other observations.

5. Declaration Form prescribed u/s 8 of the Citizenship Act, duly attested by the Embassy of India, Abu Dhabi (An Indian citizen with passport and valid visa to accompany the applicant for signing the Declaration Form).

6. Personal appearance of applicant is compulsory to establish physical identity.

7. Payment of penalty, if any, to be checked by the Embassy in terms of existing relevant instructions.

8. Citizenship letter from the concerned Embassy/Consulate attested by Ministry of Foreign Affairs, UAE (Original & Copy).
